Who follows this notice
This notice applies to Advanced Spinal Solutions LLC, doing business as Back to Total Health West Hollywood, and its workforce, including employees, staff, contractors, and other personnel involved in your care or practice operations.
Coordinating care between locations
When Back to Total Health Palm Springs is involved in your treatment, consultation, follow-up, or a visit you request, relevant health information may be shared to coordinate your care. HIPAA permits healthcare providers to exchange protected health information for treatment purposes without a separate written authorization. Other uses and disclosures remain subject to this notice and applicable law.
Our responsibilities
Back to Total Health West Hollywood is required to:
- Maintain the privacy and security of protected health information
- Provide this notice describing legal duties and privacy practices
- Follow the notice currently in effect
- Notify affected individuals following a breach of unsecured protected health information when required
- Respect your rights regarding your health information
How medical information may be used and disclosed
Protected health information may be used or disclosed without written authorization when permitted by law, including for:
- Treatment: Providing, coordinating, or managing healthcare and communicating with other providers involved in your care.
- Payment: Billing, claims, eligibility, utilization review, collections, and obtaining payment for services.
- Healthcare operations: Quality review, training, licensing, compliance, auditing, legal services, credentialing, and practice administration.
- Appointment reminders and care communications: Contacting you about appointments, treatment alternatives, or health-related services.
- People involved in your care: Sharing relevant information with a family member, representative, or other person involved in care or payment when permitted and consistent with your preferences.
- Public health and safety: Reporting or preventing disease, injury, abuse, neglect, product problems, or a serious threat to health or safety.
- Legal requirements: Responding to lawful court orders, subpoenas, oversight activities, workers’ compensation matters, law enforcement requests, or other duties required by law.
- Decedents and specialized functions: Disclosures to coroners, medical examiners, funeral directors, correctional institutions, or authorized government functions when lawful.
Uses requiring authorization
Other uses and disclosures generally require your written authorization, including most uses for marketing, sale of protected health information, and uses not otherwise permitted by law. You may revoke an authorization in writing except to the extent action has already been taken in reliance on it.
Your rights
Subject to legal limitations, you have the right to:
- Inspect or obtain a copy of medical and billing records
- Request correction or amendment of information you believe is incomplete or incorrect
- Request an accounting of certain disclosures
- Request restrictions on certain uses or disclosures
- Request confidential communications by a specific method or at a specific location
- Obtain a paper or electronic copy of this notice
- Choose a personal representative authorized to act on your behalf
- File a privacy complaint without retaliation
Requests may need to be submitted in writing, and reasonable cost-based fees may apply where permitted.
Changes to this notice
Back to Total Health West Hollywood may change this notice and make the revised notice effective for information already maintained as well as information received in the future. The current notice will be available at the practice and on this website.
